Notice of Data Privacy Incident
East Side Health District (“ESHD”) is committed to protecting the privacy and security of the information in our care. On February 2, 2024, we began mailing notification letters to certain ESHD patients whose information may have been involved in an incident that disrupted the operations of some of our IT systems.
On December 8, 2023, we learned of the incident and immediately took steps to secure our systems. We also launched an investigation with the assistance of third-party forensic experts and notified law enforcement. Our investigation determined that an unauthorized party accessed some of our systems between December 1, 2023, and December 8, 2023, and accessed or removed certain files. We then initiated a review and analysis of those files to determine their contents, which is ongoing.
Through the ongoing analysis, we determined the files contained information belonging to certain patients. The information varies per patient but includes some or all of the following: names, contact information, dates of birth, treatment and diagnosis information, test results, prescription information, dates of service, provider names, and/or health insurance information. Some patients may have also had Social Security numbers and/or driver’s license numbers involved.
We apologize for any concern this may cause. Patients whose information may have been involved are encouraged to review the statements they receive from their healthcare providers and health insurers. If they see services they did not receive, they should contact the provider or insurer who issued the statement immediately. Additionally, we are offering complimentary identity monitoring services through IDX, a ZeroFox Company, to patients whose Social Security numbers or driver’s license numbers may have been involved.
We want to assure you that we take this matter very seriously. To help prevent something like this from happening again, we have implemented additional safeguards and technical security measures to further protect and monitor our systems.
We also established a dedicated, toll-free call center to help answer questions about the incident. The call center can be reached at 1-800-939-4170, Monday through Friday from 8:00 am to 8:00 pm Central Time.
